The Case Manager I – Adult Medicine works as part of a multidisciplinary health care team to provide comprehensive, patient‑centered services in a community health setting. This role supports adult primary care patients through assessments, care planning, case management, appointment coordination, follow‑up of labs and referrals, documentation, patient education, counseling, and outreach to promote continuity of care and improved health outcomes. Reports to: RN Supervisor To perform effectively in this position, the employee must have: High school diploma or General Educational Development (GED) certificate, and completion of a State-approved program for Medical Assistants. Completion of state-approved training and certification (Calif. Business & Professional Code 2069, 2070; and Calif. Code of Regulations 1377, 1377.1) relating to the performance of injections, skin testing, and withdrawal of blood. Case Manager I – Adult Medicine position requires one (1) year minimum experience with Adult Medicine or Family Practice, counseling, health education, client advocacy, and community networking. Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy. Demonstrated competency in understanding and addressing population‑specific needs when serving adult medicine patients. Must maintain or exceed competency levels as established by corporate standards for clinical practice. Possesses excellent organizational skills and the ability to prioritize. Ability to work both as a team member and independently. Ability to perform a variety of tasks once trained. Willing and able to work a variable, flexible schedule, Monday through Saturday, and evenings. Fluency in English (speaks, read and write). Fluency in Spanish (speaks, interpret, read and write). Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
