*Job Summary* We are seeking a compassionate and dedicated Direct Support Professional to provide essential support and care to individuals with Intellectual Disabilities in our home settings. The ideal candidate will play a vital role in enhancing the quality of life for those they assist, ensuring that each individual receives personalized care tailored to their unique needs. This position requires a commitment to fostering independence, dignity, and respect for all clients. *Responsibilities* * Assist clients with daily living activities, including personal hygiene, meal preparation, and medication management. * Implement and monitor individualized care plans to meet the specific needs of each client. * Provide support in social and recreational activities to promote community engagement and personal development. * Maintain accurate documentation of client progress, incidents, and any changes in health status. * Monitor vital signs and report any significant changes to healthcare professionals as necessary. * Collaborate with family members, social workers, and other healthcare providers to ensure comprehensive care. * Assist clients with mobility and transportation needs as required. * Provide first aid when necessary and ensure a safe living environment for all clients. *Skills* * Proficient in First Aid techniques and CPR certification is required. * Experience in direct support or home community care settings is highly desirable. * Knowledge of developing and implementing care plans tailored to individual needs. * Ability to monitor vital signs accurately and respond appropriately to health concerns. * Experience or training in Intellectual Disability care is an asset. * Background in social work or working with individuals with disabilities is beneficial. * Familiarity with assisted living environments and best practices for client support. * Strong interpersonal skills with the ability to build rapport with clients and their families.
