Home health and Hospice serving LOS ANGELES and SAN BERNARDO COUNTIES . Looking for well skilled, passionate Field Registered nurses, LVNS to provide care at home. Must have home health experience and be familiar with Medicare/JCAHO guidelines. RNs will be responsible for following a physician established plan of cares under the direction of the Director. Must have Exceptional Assessment and Documentation skills Must be experienced in IV infusions, wound care, G-Tube, TPN, PICC lines, Ostomy Care , etc. All candidates must be reliable and paperwork must be turned in on a timely manner. Looking for compassionate, skilled LVNs to perform nursing duties within the LVN scope of practice and under physician's orders. Assess, treat and educate patients/care givers. Will be responsible to perform wound cares, injections, Foley care, etc. LVNs will be working directly with Patients and office Case Managers. Must be organized, have excellent people's skills as well as communication skills. Responsible for timely and accurate completion of client notes, reports, and other documentation related to the treatment and progress of clients. (PER DIEM) Licensed candidates must have the following: Current Nursing License (RN's, LVN's , must have at least 1 year experience, no NEW grads please) Current CPR Card Current Driver License Proof of Social Security or Legal Work Permit Current Physical and TB Test Current Auto Insurance Job Type: Contract Pay: $40.00 - $120.00 per hour Benefits: * Flexible schedule Medical Specialty: * Home Health * Hospice & Palliative Medicine Physical Setting: * Outpatient Experience: * Home health : 1 year (Required) Language: * Vietnamese/CANTONESE/MANDARIN/SPANISH (Preferred) Willingness to travel: * 100% (Required) Work Location: On the road
