This a culture of winning. At TMS we breed winners. Responsibilities Coordinate and oversee patient care plans in-home, ensuring timely care delivery. Assess patient needs, create and adjust care plans based on progress and metrics. Monitor clinical indicators and document outcomes to drive improvement. Meet or exceed individualized targets and contribute to team-based metrics. Collaborate with physicians, therapists, and families to align goals and care. Ensure compliance with regulations, policies, and safety standards in home health. Identify and escalate risks; implement preventive measures to maintain quality care. Document all interactions and care activities within electronic health records accurately. Utilize pipelines and territory data to forecast caseload and target outcomes. Provide patient and caregiver education to enhance self-management and adherence. Participate in performance reviews and contribute to professional development plans. Experience/Qualifications RN license valid in the relevant state with Home Health certification. Proven home health care management experience coordinating patient care plans. Strong clinical assessment skills and ability to adjust plans based on progress. Compliance-focused with knowledge of home health regulations and safety standards. Proficiency with electronic health records and accurate documentation. Experience using pipelines and territory data to forecast caseloads. Excellent communication with physicians, therapists, families; interdisciplinary collaboration. Target-driven mindset with measurable performance metrics and accountability. Ability to educate patients and caregivers to improve self-management and adherence. Contract position availability with on-site work and reliability to meet targets. What's in it for you?
