*Job Overview* Heavens Hands Home Care is looking for a dedicated CNA and Home Care Aide to join our team, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes or assisted living environments. This rewarding position allows you to make a meaningful difference in the lives of seniors, individuals with disabilities, and those requiring specialized memory or hospice care. Your energetic and attentive approach will ensure clients receive the highest quality support, promoting independence, safety, and well-being every day. *Duties* * Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support * Monitor and observe clients health status and report any changes in condition * Provide medication reminders and assist with medication administration following proper protocols and documentation standards * Support clients with specialized needs such as dementia, Alzheimer’s, or developmental disabilities through tailored care strategies and compassionate engagement * Operate care equipment such as Hoyer lifts for safe transfers, wheelchairs, gate belt, etc * Prepare nutritious meals and assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment * Document all patient interactions and medical information in compliance with HIPAA regulations, ensuring confidentiality at all times *Requirements* * Proven experience in home care , senior care, assisted living, nursing homes, or long-term care environments * Certification in CPR and first aid preferred; additional certifications such as hospice care or dementia training are preferred * Ability to work independently while following detailed care plans and protocols * Strong patient observation skills to identify changes in care status promptly * Experience working with individuals with disabilities or developmental challenges is highly valued * Comfortable using systems for accurate recordkeeping and communication * Physical ability to perform caregiving duties including lifting with Hoyer lifts, standing for extended periods, and assisting with mobility tasks Join us to provide exceptional home care that enhances quality of life!
