Are you a compassionate, reliable Certified Home Health Aide looking to make a meaningful difference in the lives of patients? East West Home Health is seeking dedicated CHHAs to join our clinical team. In this role, you will provide essential personal care and emotional support to patients in their own homes. We are actively hiring clinicians to service patients in *Cerritos* and neighboring Southern California communities (including Artesia, Norwalk, Lakewood, and La Palma). As a member of our team, you will collaborate closely with our Director of Patient Care Services (DPCS) and Registered Nurse Case Managers to ensure the highest standards of safety, infection control, and patient satisfaction. *Key Responsibilities* * Provide high-quality personal care, including bathing, grooming, dressing, and oral hygiene. * Assist patients with safe transfers, ambulation, and prescribed exercises. * Measure and record vital signs (temperature, pulse, respiration) when requested by the supervising nurse. * Maintain a clean, safe, and healthy patient environment (including basic light housekeeping related to patient care). * Report any sudden changes in the patient’s physical condition, behavior, or safety hazards to the supervising RN Clinical Manager immediately. * Adhere strictly to infection control standards (e.g., proper hand hygiene and bag technique). * Document all care provided and submit clinical visit notes accurately and timely within the agency's mandated *24-to-48-hour turnaround window*. *Job Requirements & Qualifications* * *Certification:* Current, active Certified Home Health Aide (CHHA) certification issued by the California Department of Public Health (CDPH). * *Experience:* Minimum of 1 year of home health or related hospice/vulnerable care experience preferred. * *Health & Safety Clearance:* * Valid physical exam completed within the last 12 months. * Clear Tuberculosis (TB) screening (2-step PPD, blood test, or chest X-ray with symptom sheet). * Current CPR Certification (Basic Life Support for Healthcare Providers). * *Background Check:* Must pass a criminal background check and clear monthly OIG/GSA exclusion screenings. * *Transportation:* Valid California Driver’s License and current automobile insurance with an operable vehicle. * *Competency:* Must complete and pass the agency’s initial skills competency validation. *Why Join East West Home Health?* * *Flexible Schedules:* Perfect for per-diem flexibility—choose a caseload that fits your lifestyle. * *Local Commutes:* Serve patients right in your neighborhood, centering around the Cerritos/Artesia area. * *Strong Clinical Support:* Work under a highly supportive management team with a structured QAPI program focused on patient safety and documentation excellence. * *Compliance Minded:* We hold ourselves to rigorous CHAP accreditation standards to ensure a professional, secure working environment for all staff.
