*Overview* We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in various EMR/EHR charting systems such as OASIS. This role involves providing high-quality patient care across multiple settings, including long-term care, home health care, and community health environments. The LVN will be responsible for performing comprehensive assessments, supporting patient recovery and well-being through expert nursing practices. Experience in home health care is preferred. *Responsibilities* * Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations. * Document all patient interactions accurately using EMR/EHR charting systems such as OASIS. * Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients. * Provide education on medication administration, disease management (e.g., diabetes or hypertension), nutrition, and health coaching to patients and their families. * Support infection control practices and ensure compliance with HIPAA regulations to protect patient confidentiality. * Respond effectively to urgent situations. *Requirements* * Valid LVN license in the state of practice with current certification. * Proven experience in diverse clinical settings such as home health care, hospice care, and nursing homes. * Demonstrated proficiency with EMR/EHR charting systems like OASIS. * Knowledge of medical terminology. * Strong clinical skills in areas such as wound care and specimen collection. * Ability to work independently while collaborating effectively within multidisciplinary teams. * Excellent communication skills with the ability to educate patients on treatment plans and health management strategies. * We value experienced nurses committed to delivering compassionate care while adhering to the highest standards of safety and professionalism.
