Job Description
*Overview* We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to clients with diverse needs. This role offers an opportunity to make a meaningful difference in patients' lives by delivering personalized support and assistance in a variety of settings, including assisted living, nursing homes, and community environments. The ideal candidate will possess strong patient observation skills, and familiarity with EMR/EHR charting systems. Certification as a Home Health Aide is required, along with a commitment to maintaining patient privacy and safety. *Duties* * Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support * Monitor and document vital signs and patient conditions accurately using EMR/EHR systems * Maintain detailed medical documentation in compliance with HIPAA regulations * Observe and report changes in patient health or behavior promptly to healthcare professionals * Work collaboratively with healthcare teams to deliver comprehensive home & community care *Requirements* * Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) * Experience working in home health, long-term care, nursing homes, or assisted living facilities preferred * Knowledge of medical terminology and medical records management * CPR certification required; additional certifications such as HIPAA training are advantageous * Ability to work independently while following care plans and protocols * Strong communication skills and compassionate bedside manner * Ability to work flexible hours * Must pass background checks and adhere to all safety protocols including infection control standards Join our team to provide compassionate, high-quality care that enhances the well-being of our clients while advancing your career in home health services.
