GLOBAL HOME HOSPICE SERVICE INC is looking for a Per Diem Licensed Vocational Nurse for Continuous Care Shifts, a member of the home care team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the client’s needs. The team is looking for a dedicated individual with a passion for care and values honesty and commitment. The candidate must be willing to work 8 to 12 hour shifts when needed. *Responsibilities:* * Provides direct patient care as defined in the California State Nurse Practice Act * Manage a patient’s symptoms during a short term medical crisis. * Recognize and interpret symptoms, institute remedial measures and report significant changes in the patient condition immediately. * Chart the patient’s condition hourly to provide accurate and timely documentation of patient services to reflect the plan of care * Implement current nursing practice following a comprehensive assessment and the plan of care. * Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care. * Uses equipment and supplies effectively and efficiently. * Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications. * Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities. * Performs other duties as assigned by the registered nurse. * Should be able to travel to other areas. * Provide accurate and timely documentation of patient services to reflect the plan of care. * Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. * Uses equipment and supplies effectively and efficiently. * Calls the Medical Director if there is a new order for medications. * Writes the doctor's order and order the medication from the Pharmacy. *Requirements:* * Graduated from an accredited Vocational Nurses Program. * Hospice experience preferred. * Currently licensed as a LVN in the State of California. * Demonstrates good verbal and written communication and organizational skills. * CPR * California Driver's License * Auto Insurance * Understands philosophy of the hospice concept. Also understand needs of the terminally ill. * Physical Exam, TB Test or Chest Xray * Professional Liability Insurance Job Types: Part-time, Contract Pay: $30.00 - $40.00 per hour Experience: * hopsice: 2 years (Required) Language: * Spanish (Preferred) License/Certification: * LVN (Preferred) Work Location: Multiple locations
