*Director of Patient Care Services (DPCS) – RN | Home Health | Los Angeles, CA* Beverly Glenn is a respected healthcare recruiter partnered with two of Southern California's largest home health and hospice agencies. Our partner agencies are licensed, Medicare-certified, and CHAP accredited, delivering exceptional care throughout California. We are seeking an experienced Registered Nurse to step into a Director of Patient Care Services (DPCS) role with a team committed to enhancing patients' quality of life through individualized, in-home care. *About the Role* As DPCS, you will oversee nursing services and work collaboratively with physicians to develop and execute individualized treatment plans. You will lead with clinical excellence, guide documentation standards, and serve as a key contributor to interdisciplinary care planning. *Required Qualifications* * Graduate of a Board of Registered Nursing–approved nursing program with a current, active California RN license * Minimum 5 years of clinical nursing experience; home health or hospice experience strongly preferred * Strong clinical judgment, analytical skills, and documentation accuracy; able to work with minimal supervision * Excellent organizational skills; physically able to walk, bend, stoop, and lift up to 50 lbs * Valid California driver's license with a reliable, insured vehicle in good working condition * Computer literate and comfortable adopting agency technology for documentation and team communication *Essential Duties and Responsibilities* * Direct nursing services in accordance with physician-ordered treatment and care plans; observe and evaluate patient response and update plans as needed * Assess each patient's rehabilitative status and develop, modify, and update individualized treatment plans on an ongoing basis * Set patient goals that are reasonable, necessary, and skill-appropriate; participate in interdisciplinary planning, case conferences, and staff meetings * Communicate with physicians to clarify orders, report significant changes, and recommend treatment modifications * Observe, record, and report patient condition accurately to the attending physician and in the medical record * Prepare and submit all clinical documentation — including assessments, care plans, visit notes, medication records, progress summaries, CHHA supervisions, and discharge summaries — within agency-established timelines *Job Type:* Full-Time _Equal opportunity employer.
