We have been steadily growing since 2007 and our goal is to challenge and retain the members of our organization. _*Hospice Field LVN*_ *Job Description:* * Provides nursing services, treatments and diagnostic and preventive procedures as assigned. * Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition. * Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. * Maintains accurate and complete records of observations, treatments and care of patient * Prepares clinical and progress notes. * Assists the physician and RN in performing specialized procedures. * Prepares equipment and materials for treatments. * Observes aseptic technique as required. * Assists the patient/family in learning appropriate self-care techniques. _*Hospice Field LVN*_ (Care Coordinator) * Assists the patient/family in learning appropriate self-care techniques. * Initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications, and continuing care requirements. * Care Coordinator must develop interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers * Must evaluate outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team; implementing team recommendations * Be able to communicate with patient and family members, different members of interdisciplinary team, pharmacies, DME company, and more. *Skills: (Care Coordinator) * - Proven working experience in care coordination, including as a nurse or medical health care manager or a related job - Excellent knowledge of case management principles - Effective communication skills - Compassionate with teamwork skills *Requirements:* * Drivers License * Auto Insurance * CPR/BLS Certification * Physical and TB Test Job Types: Full-time, Part-time Pay: $35.00 - $40.00 per hour Medical Specialty: * Home Health Work Location: In person
