Job Title Weekend Licensed Vocational Nurse (LVN) Company Overview Anchor Co-Living partners with managed care plans across California to provide Recuperative Care, Short-Term Post-Hospitalization Housing, and related CalAIM Community Supports and Enhanced Care Management services. Our homes help bridge the gap between hospital discharge and longer-term stability by giving residents a safe place to recover while addressing the medical and social factors that affect healing. This role helps ensure residents receive consistent clinical support during a critical stage of recovery. The Opportunity Anchor Co-Living is hiring a Weekend Licensed Vocational Nurse (LVN) to provide on-site nursing support in a non-acute residential setting in Los Angeles. This role is specifically focused on weekend coverage and is ideal for an LVN who wants meaningful clinical work in a relationship-driven, community-based environment. You will support residents recovering from illness, injury, or hospitalization while working closely with care coordinators, case managers, and the broader care team to promote recovery, stability, and safe transitions. There may also be additional per diem weekday shifts available based on program needs. What You'll Do Provide weekend nursing coverage for residents in recuperative care and short-term post-hospitalization housing. Administer medications, perform wound care, monitor vital signs, and observe changes in resident condition. Support residents with daily living needs in a respectful, compassionate, and professional manner. Communicate changes in condition and coordinate with the care team to support timely follow-up and continuity of care. Maintain accurate, timely documentation that meets clinical, program, Medi-Cal, and managed care plan standards. Help create a safe, clean, and supportive environment during weekend shifts. What Success Looks Like Within the first 6 to 12 months, success in this role will look like: Residents receive reliable, high-quality nursing care during weekend coverage. Medication administration, wound care, and routine monitoring are completed accurately and on time each weekend shift. Changes in condition are identified promptly and communicated clearly to support appropriate follow-up. Documentation is complete, timely, and compliant across all weekend nursing activities. Residents are better supported in their recovery and transition planning because of consistent clinical care and coordination. What We're Looking For We are looking for an LVN who brings sound clinical judgment, reliability, and a calm, compassionate presence. You should be comfortable working in a residential recuperative care environment and confident providing weekend coverage with strong follow-through and professionalism. An active California LVN license in good standing and current CPR/BLS certification are required. Two or more years of direct patient care experience in home health, skilled nursing, recuperative care, or a similar setting is preferred. Experience supporting individuals with Alzheimer's, dementia, or chronic illness is a plus. You should also be physically able to assist residents with mobility and transfers and work well as part of a collaborative team. Compensation and Benefits Pay: $30-$36 per hour. Schedule Type: Weekend coverage with possible per diem weekday shifts based on program needs. Work Location / Schedule Location: On-site in Los Angeles, California 90007. Schedule: Weekends required. This position is primarily for weekend coverage, with possible per diem weekday shifts as needed. Candidates must have reliable transportation or a dependable commute to the site.
