*Job Summary* The LVN provides direct nursing care and clinical oversight to residents in Congregate Living Health Facility or Home Health. The LVN delivers safe, effective care within LVN scope of practice under the supervision of an RN and in accordance with Title 22 regulations, physician orders, facility policies, and individualized care plans. ESSENTIAL DUTIES & RESPONSIBILITIES 1. Clinical Care & Nursing Responsibilities Administers medications safely, including oral, topical, subcutaneous, enteral, and IM routes, in accordance with physician orders and facility policy. Performs skilled nursing tasks within LVN scope, including: Wound care and dressing changes Tracheostomy care and suctioning (if applicable) Oxygen and respiratory treatments Enteral feeding (GT/JT) and flushes Blood glucose monitoring and insulin administration Catheter care and bowel/bladder management Conducts comprehensive shift assessments and documents findings accurately. Monitors residents for changes in condition and immediately reports abnormalities to the RN or provider. 2. Documentation & Communication Completes timely and accurate documentation in medical records, MAR/TARs, nursing notes, and 24-hour reports. Communicates effectively with interdisciplinary team, families, and providers while maintaining resident confidentiality. Participates in care plan development, updates, and reviews; ensures care is carried out appropriately. 3. Supervision & Leadership Provides direction to CNAs/Direct Care Staff ensuring proper delivery of ADLs, safety, and resident support. Ensures staff follow infection control, standard precautions, and facility procedures. Promotes a positive, respectful, and team-oriented work environment. 4. Safety & Compliance Maintains compliance with Title 22, CLHF regulations, facility policies, and scope of practice standards. Ensures safe medication practices, proper storage, and narcotic accounting per policy. Responds to emergencies promptly, initiates basic life support (if certified), and follows emergency procedures. Ensures resident rooms and equipment are clean, safe, and functioning. 5. Resident Support & Advocacy Assists with ADLs as needed to ensure resident well-being. Provides education to residents and families regarding treatment plans, medications, and facility procedures. Advocates for resident rights, safety, comfort, and dignity at all times. QUALIFICATIONS Current and unrestricted LVN license in the State of California. Current CPR/BLS certification. Prior experience in subacute, long-term care, home health, or CLHF preferred. Ability to lift, transfer, and reposition residents safely. Strong communication, documentation, and critical thinking skills. Ability to work independently in a small, high-acuity environment. WORKING CONDITIONS CLHF/ Home Health with medically fragile residents, trachs, Vent, oxygen GT/JT, chronic illness, or rehabilitation needs depending on census.
