This paid position offers an opportunity to grow your skills in hospice care while making a positive impact every day. *Duties* * Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to promote independence and comfort * Monitor and document patient observations, including vital signs, medication administration, and changes in condition using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems * Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities * Provide emotional support and companionship to patients, fostering a calming environment during difficult times * Assist with medical procedures including catheter care, Hoyer lift transfers, and basic wound care as directed by nursing staff * Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control protocols are followed in compliance with HIPAA regulations * Collaborate closely with nursing staff to ensure comprehensive patient care and accurate medical documentation *Requirements* * Prior experience in hospice care, nursing homes, assisted living facilities, or home health is highly preferred * Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation * Certification in CPR is required; additional certifications such as medication administration or catheter care are a plus * Experience working with seniors or individuals with disabilities or chronic conditions such as Alzheimer’s or dementia enhances your effectiveness in this role * Ability to lift safely using equipment like a Hoyer lift and perform patient transfers while maintaining safety standards * Strong communication skills to effectively observe patient needs and document findings accurately * Compassionate attitude with a commitment to providing respectful, patient-centered care Join us in delivering exceptional hospice services where your caring nature transforms lives.
