HHHS provides its employees with flexibility, competitive wages, unbeatable health benefits, and a positive work environment. * *Competitive Health Benefits* * *This is a Full-time Case management Position Monday through Friday from 9:00 to 5:30PM- on site* * *We are looking for a Registered Nurse(Case Manager) * *Job Functions / Responsibilities* * Plans, Organizes and Directs home care services * Completes an initial assessment of patients to determine home care needs * Regularly re-evaluates the patient's nursing needs. * Initiates a plan of care and makes necessary revisions as patient status changes * Counsels the patient and family in meeting nursing and related needs * Provides health care instructions to the patient as appropriate per assessment and plan of care * Communicates with the physician regarding the patient's needs; obtains/receives physician's orders as required *Who We Are* Hollywood Home Health Services is an industry-leading skilled nursing agency based in Southern California.
