_*Pay Rate: $30-36/hour. Mileage reimbursed. *_ _*Per Diem positions available. *_ *Coverage area includes: *Los Angeles, San Gabriel Valley, Pasadena, Alhambra, Whittier, surrounding cities. The hospice LVN cares for terminally ill patients in their home under the supervision of an RN and adhering to an individualized plan of care. This includes implementing and educating both patients and families on end of life process, medication administration and safety, safety in the home, treatments per MD order, etc. * Needs to be organized to manage patients * Needs to communicate well with administrative and field staff * Needs to document appropriately and follow company policy and procedure * *MUST* have experience working with facilities. *Required work experience:* * Experience in Hospice care LVN *Required Qualifications:* * Currently licensed as a LVN in the State. * Minimum of 1 year nursing experience. * Current BLS provider card * Great verbal and written communication, and organization skills. * Must be a licensed driver with an automobile that is insured in accordance with state requirements * Basic computer competence *Preferred Qualifications:* * Bilingual (English and Spanish) preferred *Our Core Values:* * _*Effective communication*_ * _*Adaptability*_ * _*Accountability*_ * _*Friendly Environment*_ Pay: $30.00 - $36.00 per hour Benefits: * Employee assistance program * Employee discount * Flexible schedule Medical Specialty: * Geriatrics * Home Health * Hospice & Palliative Medicine Physical Setting: * Long term care * Nursing home * Outpatient People with a criminal record are encouraged to apply Experience: * Hospice: 1 year (Preferred) * Nursing: 1 year (Preferred) Language: * Spanish (Preferred) License/Certification: * LVN (Required) * BLS Certification (Required) Shift availability: * Day Shift (Required) Ability to Relocate: * Alhambra, CA: Relocate before starting work (Required) Willingness to travel: * 75% (Required) Work Location: On the road
