We are seeking a reliable nurse to join our team for a consistent weekday evening case in Hawthorne. This is a great opportunity to build rapport with a long-term patient while providing skilled, specialized care in a supportive environment. *Patient Overview* We are seeking an LVN for a home health case involving a 25-year-old adult patient with a G-tube (Gastrostomy Tube). The patient requires ongoing skilled nursing support and monitoring in the home setting. *Responsibilities* * Provide skilled nursing care in the home environment * Manage and monitor G-tube care and feedings * Perform routine patient assessments and documentation * Follow the physician’s plan of care * Monitor patient condition and report any changes promptly * Ensure patient safety and comfort at all times * Complete accurate and timely electronic charting * Communicate effectively with the care team and family as needed *Qualifications* * Active California LVN * Current CPR certification * Experience with G-tube care preferred * Strong clinical assessment and documentation skills * Ability to work independently in a home setting * Reliable, compassionate, and professional demeanor *Schedule* * Monday – Friday * 4:00 PM – 10:00 PM * Evening shifts (30 hours per week) *Benefits* * 401(k) * Dental insurance * Health insurance * Vision insurance * Referral program *Pay* Competitive hourly rate based on experience *Work Location* * Hawthorne, CA * In-person home health setting Pay: $28.00 - $30.00 per hour Benefits: * 401(k) * Dental insurance * Health insurance * Referral program * Vision insurance Work Location: In person
