Job Summary We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Home Health team. The Home Health Nurse is responsible for providing skilled nursing care to patients in their homes, promoting health, preventing disease, and assisting patients in achieving optimal independence and quality of life. The LVN will work collaboratively with physicians, therapists, caregivers, and other healthcare professionals to develop and implement individualized plans of care. LVN who can cover Long Beach, Ca and neighboring cities. Essential Duties and Responsibilities * Provide skilled nursing services in accordance with physician orders, individualized care plans and agency policies. * Monitor and evaluate patient condition, progress, and response to treatment. * Wound care, catheter care, and other nursing interventions as ordered. * Educate patients and caregivers regarding disease management, medications, safety precautions, and self-care techniques. * Coordinate care with physicians, therapists, social workers, and other healthcare team members. * Accurately complete and submit all required clinical documentation in a timely manner. * Identify changes in patient condition and promptly report findings to the physician and appropriate team members. * Participate in case conferences and interdisciplinary team meetings. * Ensure compliance with Medicare, state, federal, and agency regulations. * Promote patient safety and infection control practices during all visits. Qualifications * Current and unrestricted Licensed Vocational Nurse (LVN) in the State of California. * Home Health experience preferred but not required. * Current CPR/BLS certification. * Valid driver's license, reliable transportation, and current auto insurance. * Strong assessment, communication, and organizational skills. * Ability to work independently and manage time effectively. * Knowledge of Medicare home health regulations preferred. * Bilingual English and Spanish is preferred.
