*Job Summary* We are seeking a dedicated and compassionate Caregiver / Home Care Aide (HCA) to provide exceptional support to individuals in their homes and community settings. This role involves delivering personalized care that promotes independence, safety, and well-being for clients with diverse needs, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer’s or dementia. The ideal candidate will possess a strong background in patient observation, medication assistance, and activities of daily living (ADLs), ensuring a respectful and nurturing environment. *Responsibilities* * Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. * Monitor patients’ health status by observing vital signs, reporting changes, and maintaining detailed care records in compliance with HIPAA regulations. * Provide medication reminders and assist with medication administration under supervision or according to care plans. * Prepare nutritious meals tailored to dietary restrictions and preferences while maintaining proper food safety standards. * Support clients with housekeeping tasks including cleaning, laundry, and maintaining a safe living environment. * Implement individualized care plans designed by healthcare professionals to meet each client’s unique needs. * Engage clients in social activities and companionship to promote mental stimulation and emotional well-being. * Maintain thorough documentation of patient progress, incidents, and observations for team communication and ongoing care adjustments. *Experience* * Prior experience working as a caregiver or home health aide in settings such as assisted living facilities, nursing homes, or group homes is highly desirable. * Demonstrated knowledge of patient care techniques including assisting with ADLs, medication management, and vital signs monitoring. * Familiarity with specialized care such as Alzheimer’s/dementia care, hospice support, or working with individuals with developmental disabilities or autism spectrum disorder. * Experience operating medical equipment like Hoyer lifts or other mobility aids is advantageous. * Knowledge of HIPAA regulations ensuring confidentiality and privacy of patient information. * Proven ability to handle heavy lifting safely while supporting residents’ mobility needs. * Strong interpersonal skills to work effectively with diverse populations including seniors and individuals with disabilities.
