Job Description
We are seeking a compassionate and dedicated Home Health Nurse to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in their homes while promoting comfort, independence, and recovery. *Responsibilities:* * Deliver skilled nursing care during home visits * Monitor patient health and update care plans * Educate patients and caregivers * Coordinate with healthcare providers * Maintain accurate patient documentation *Qualifications:* * Active RN license * CPR/BLS certification * Strong clinical and communication skills * Home health experience preferred * Ability to travel to patient homes * Knowledgeable in Data Soft Logic (DSL) EMR.
