*Job description:* Licensed Vocational/Practical Nurse (LVN/LPN) – Home Health Salary: From $40.00 per hour Schedule: Monday to Friday | Day Shift | On-Call Availability Specialties: Home Health Millennium Home Health is Highly in need of Compassionate and Reliable LVNs/LPNs! Join our dedicated team at Millennium Home Health as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). You’ll play an essential role in supporting patient health and well-being under the supervision of a Registered Nurse. Key Responsibilities (include but are not limited to): * Deliver direct, hands-on patient care in the home setting. * Implement the plan of care initiated by the RN or provider. * Ensure accurate and timely documentation in line with the plan of care. * Educate patients and families/caregivers with information relevant to diagnoses and care plans. * Coordinate with other members of the healthcare team and report needs to the RN or Clinical Supervisor. * Use equipment and medical supplies effectively and efficiently. * Participate in professional growth and development activities. * Perform other duties as assigned by the supervising RN. * Maintain patient confidentiality and adhere to HIPAA regulations. * Supervise medical assistants when needed. * Maintain a safe, clean working environment and follow infection-control protocols. _Note: The above statements are intended to reflect the general duties and responsibilities of this position.
