*Job Summary* Join our dynamic healthcare team as a Home Health Registered Nurse, where your expertise will directly impact patient recovery and well-being in the comfort of their own homes. In this vital role, you will deliver personalized nursing care, conduct comprehensive assessments, and coordinate with multidisciplinary teams to ensure optimal health outcomes. This position offers an exciting opportunity to apply your clinical skills in a flexible, community-focused environment that values proactive and positive patient engagement. *Responsibilities* * Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop individualized care plans. * Administer medications, injections, IV infusions, and perform wound care procedures such as suturing and ostomy care following aseptic techniques. * Manage complex medical conditions including dialysis, cardiology-related treatments like catheterizations, and acute pain management strategies. * Utilize electronic health record (EHR) systems such as Epic or Cerner to document patient interactions accurately and efficiently. * Provide education on disease management, medication adherence, nutrition, and health coaching tailored to each patient's needs. * Coordinate discharge planning and collaborate with physicians, therapists, social workers, and family members to ensure seamless continuity of care. * Conduct triage over the phone or in person to assess urgent needs and determine appropriate interventions or referrals for patients with infectious diseases or behavioral health concerns. *Experience* * Valid RN license with current state registration; additional certifications in Basic Life Support (BLS) required. * Proven experience in home care settings or medical-surgical units demonstrating strong patient assessment skills. * Critical care experience including ICU or emergency room background highly preferred for managing acute conditions. * Familiarity with EMR/EHR systems such as Epic or Cerner; proficiency in ICD coding (ICD-10/ICD-9) is advantageous. * Experience working with individuals with disabilities, memory care needs, or in hospice & palliative medicine enhances your ability to provide compassionate care. * Knowledge of infection control protocols, sterile processing techniques, and laboratory specimen collection ensures safe practice standards. * Ability to adapt quickly to diverse environments such as assisted living facilities, nursing homes, or outpatient clinics while maintaining high-quality patient care.
