*Job Summary* We are seeking a compassionate and dedicated Spanish speaking Registered Nurse for our Home Health Care Inc. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong commitment to patient-centered care and the ability to work independently while collaborating with a multidisciplinary team. *Responsibilities* * Assess patient health needs and develop individualized care plans. * Administer medications and treatments as prescribed, including IV infusions. * Provide specialized care such as ostomy care, infant care, and long-term care support. * Monitor patients' conditions, including those requiring neonatal ventilator support. * Educate patients and families on health management, including health coaching techniques. * Collaborate with healthcare professionals to coordinate comprehensive care for individuals with developmental disabilities or those needing botulinum toxin treatment. * Maintain accurate patient records and documentation in compliance with healthcare regulations. * Ensure a safe and supportive environment for patients during home visits. *Experience* home health: 3 year (Required) License: RN (Required) *SOC Oasis Visit * *ROC, RC, DC Oasis Visit * *IV Visit * *Follow up Visit * *Sup Visit / Eval / DC non Oasis Visit* *RN MUST BE PUNCTUAL WITH COMPLETING PAPERWORK.* *Home Health Case Management skills:* * Nursing Assessments * Medication Reconciliations * Work independently/ Time management * Ability to manage patient supplies * Ability to work as part of a team * IV administration and blood draw experience preferred including: PICC lines Portacaths All central lines Peripheral access TPN Candidates should possess the following skills and experience: * Valid Registered Nurse (RN) license in the state of practice. * Experience in home health nursing or similar settings is preferred. * Proficiency in administering IV infusions and managing ventilated patients. * Familiarity with caring for individuals with developmental disabilities is advantageous. * Knowledge of health coaching principles and practices is a plus. * Experience in long-term care settings will be beneficial. *Coverage areas include:* *Los Angeles County* *Riverside County* *San Bernardino County* We are looking for a dedicated professional who is eager to make a difference in the lives of our patients through exceptional nursing care.
