*Part-Time Home Health LVN * *Pay:* $40–$45 per hour, depending on experience *Mileage:* Paid mileage reimbursement at the current IRS business mileage rate *Job Type:* Part-time *Location:* Santa Monica, Culver City, and nearby West Los Angeles areas *Setting:* Home Health / Patient home visits *Experience Required:* Minimum 2 years LVN experience We are looking for an experienced and dependable *Licensed Vocational Nurse (LVN)* to provide *home health follow-up visits* to patients in the *Santa Monica and Culver City area*. This is a part-time field position for an LVN who is comfortable visiting patients in their homes, working independently, completing accurate documentation, and communicating with the clinical team regarding patient status and changes in condition. The ideal candidate is reliable, professional, compassionate, and experienced in patient care. Home health experience is preferred, but we require at least *2 years of LVN experience*. Responsibilities * Visit patients in their homes for scheduled follow-up visits * Provide LVN care within scope of practice and according to the patient’s plan of care * Monitor patient condition and report changes to the supervising nurse or clinical team * Take and document vital signs * Perform treatments and nursing tasks as ordered * Reinforce patient and family education as appropriate * Complete accurate and timely visit documentation * Communicate professionally with patients, families, caregivers, and office staff * Travel locally between patient residences in Santa Monica, Culver City, and nearby areas Requirements * Active California LVN license in good standing * Minimum *2 years of LVN experience required* * Home health, hospice, private duty, or field nursing experience preferred * Current CPR/BLS certification * Valid driver’s license * Reliable transportation * Current auto insurance * Ability to travel to patient homes in Santa Monica, Culver City, and nearby areas * Strong documentation and communication skills * Ability to work independently and manage time effectively * Professional, dependable, and compassionate Compensation & Benefits * *$40–$45 per hour*, depending on experience * Paid mileage reimbursement * Part-time schedule * Local patient visits * Flexible scheduling based on patient needs * Supportive clinical supervision * Opportunity for additional visits as available Schedule Part-time position.
