As a valued member of our care team, you will assist clients with daily living activities, provide companionship and emotional support, monitor client well-being, and ensure services are delivered according to each client’s Individual Service Plan (ISP). This position is ideal for caring, dependable, and motivated individuals who are passionate about helping others thrive in their home and community environments. Duties & Responsibilities * Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, mobility assistance, and hygiene support * Provide companionship, supervision, and emotional support to clients in home and community settings * Support meal preparation, light housekeeping, laundry, and maintaining a safe living environment * Monitor and report changes in client condition, behavior, mood, mobility, or safety concerns * Follow each client’s Individual Service Plan (ISP) and company care guidelines * Accurately document daily services, incidents, and shift activities in electronic documentation systems such as CareSmartz360 * Maintain confidentiality and HIPAA compliance at all times * Respond appropriately during emergencies and follow mandated reporting procedures when required * Assist clients with mobility support and safe transfer techniques within training guidelines * Maintain professional boundaries and respectful communication with clients, families, and team members Qualifications & Skills * Previous caregiving, home care, direct support, assisted living, or healthcare experience preferred * Compassionate, dependable, patient, and professional attitude * Strong communication and interpersonal skills * Ability to work independently and as part of a team * Basic computer or mobile app proficiency for documentation and scheduling systems * Knowledge of HIPAA and confidentiality standards preferred * CPR/First Aid certification preferred * Experience supporting individuals with developmental disabilities, dementia, memory care, hospice, or senior care is a plus * Reliable transportation and punctual attendance required * Ability to pass background clearance requirements Requirements * Complete all onboarding, training, and required compliance requirements * Maintain accurate daily documentation and timekeeping records * Work only approved schedules and authorized assignments * Follow all safety, confidentiality, and company policies * Ability to safely perform physical caregiving tasks as required Benefits * Flexible scheduling opportunities * Paid sick leave in accordance with California law * Ongoing training and professional development * Supportive team environment * Meaningful and rewarding work experience * Opportunities for career growth within Taylored Care Why Join Taylored Care?
