We are seeking an experienced Registered Nurse (RN) for a primarily office-based role centered on clinical coordination, patient support, workflow oversight, and care team collaboration. This position is ideal for an RN who excels in care management, operational coordination, and team support while working in a structured administrative environment. This role focuses on overseeing patient care processes, supporting internal teams, ensuring excellent service for patients and partner facilities, and helping improve clinical workflows—all while maintaining minimal field responsibilities. Compensation & Benefits * Competitive salary based on experience * Medical, dental, and vision insurance * Paid time off * Retirement plan * Supportive leadership and collaborative team culture * Growth opportunities into leadership support roles Position Summary As an Office-Based RN, you will serve as a key clinical and operational resource by: * Supporting patient coordinators, case managers, and providers * Assisting with triage and clinical decision support * Overseeing care coordination processes * Enhancing patient and facility communication * Improving administrative workflows and team efficiency * Participating in training, coaching, and quality improvement initiatives Key ResponsibilitiesClinical Coordination & Administrative Oversight * Serve as the primary office-based clinical resource for patient coordination teams * Support triage, urgency assessment, and care escalation using established protocols * Review and coordinate patient care plans, follow-up needs, and provider communication * Ensure timely and accurate coordination between providers, patients, families, and facilities * Monitor care workflows to improve operational efficiency and patient experience * Assist leadership with process improvement and protocol development * Support training and mentoring for team members Patient & Facility Support * Provide high-quality customer service and clinical guidance * Support communication with assisted living and skilled nursing partners * Help resolve coordination issues while maintaining professional and empathetic service Limited Clinical Field Support (As Needed) * Occasional approved field visits for RN-level procedures * In-and-out catheterization * IV starts * Suture or staple removal * Other procedures within scope and training *Note:* This is not a home health or primarily field-based role. The core focus is office-based nursing support, coordination, and clinical oversight.
