This paid position offers an exciting opportunity to utilize your nursing skills across diverse clinical settings, including acute care, geriatrics, and chronic disease management. *Duties* * Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. * Administer medications via injections, IV infusions, and other routes while adhering to HIPAA regulations and aseptic techniques. * Provide specialized care for patients with complex needs such as Alzheimer’s disease, dementia, or those requiring memory care and hospice services. * Manage wound care procedures including dressing changes, catheterization, and sterile processing techniques. * Educate patients and their families on health coaching topics like nutrition, medication management, pain control, and chronic disease management. * Coordinate discharge planning and case management efforts to ensure seamless transitions between inpatient and outpatient settings. * Collaborate with multidisciplinary teams across hospital medicine, emergency medicine, ICU (Intensive Care Unit), PICU (Pediatric Intensive Care Unit), and Level I Trauma Center environments to deliver acute care and trauma support. * Conduct triage over the phone or in person to evaluate urgent needs related to infectious disease care, respiratory issues, or post-acute care requirements. * Document all patient interactions thoroughly in medical records using EMR/EHR systems such as Athenahealth or eClinicalWorks. * Assist with specimen collection/processing, phlebotomy, and diagnostic evaluations when necessary. *Skills* * Proven experience with Epic EMR systems or similar electronic health record platforms such as Cerner or Athenahealth. * Critical care experience in settings like ICU or Level I Trauma Centers is highly valued. * Strong background in long-term care environments including assisted living facilities and nursing homes; familiarity with geriatrics and dementia care is essential. * Skilled in IV infusion therapy, medication administration including injections and tube feeding management. * Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or spinal taps, and medical terminology for accurate documentation. * Experience working with individuals with developmental disabilities or behavioral health needs; adept at managing complex cases involving acute pain management or infectious diseases. * Ability to perform physical examinations and assessments involving anatomy knowledge and physiology principles. * Competence in infection control practices, airway management techniques such as ventilator support, and emergency response protocols including basic life support (BLS). * Familiarity with home & community care practices including home care visits for pediatric patients (neonatal ventilator use) or adult populations requiring hospice or palliative services. * Excellent communication skills for patient education on health coaching topics like nutrition or family planning; capable of working effectively within multidisciplinary teams across outpatient clinics or inpatient units.
