*Job Overview* We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. This role is vital in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The ideal candidate will possess strong clinical skills, a deep understanding of patient needs, and the ability to work collaboratively with healthcare providers to enhance patient outcomes. *Duties* * Conduct comprehensive assessments of patients to evaluate their medical, psychological, and social needs. * Develop individualized care plans that address the unique needs of each patient, including discharge planning and follow-up care. * Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient services effectively. * Utilize management techniques to ensure optimal resource allocation and quality care delivery. * Provide education and support to patients and their families regarding treatment options, health management, and available resources. * Monitor patient progress and adjust care plans as necessary based on ongoing evaluations and diagnostic evaluations. * Advocate for patients’ rights and ensure they receive appropriate services in a timely manner. * Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with regulatory standards. *Experience* * Current Registered Nurse (RN) license in good standing. * Proven experience in case management or utilization management within a healthcare setting. * Strong knowledge of physiology, diagnostic evaluation processes, and primary care principles. * Excellent communication skills with a focus on patient service and collaboration. * Ability to work independently while managing multiple cases effectively. * Familiarity with electronic health records (EHR) systems is preferred.
