*Certified Home Health Aide (CHHA) – Long Beach/Huntington Beach, CA* *Hourly Rate:* $27.00/hour (Full-Time) *Hospice Experience and Bilingual in Spanish Preferred* *Work Schedule:* Monday–Friday | 8:30 AM – 5:00 PM Are you a dependable and compassionate *Certified Home Health Aide* who enjoys working directly with patients and making a meaningful difference in their daily comfort and quality of life? *Agape Hospice & Palliative Care* is seeking a full-time *CHHA* to support patients throughout *Long Beach and surrounding areas*. We are a clinician-owned, Joint Commission–accredited hospice known for providing exceptional patient-centered care, strong teamwork, and a supportive work culture. This role is ideal for an aide who is reliable, compassionate, and comfortable traveling to patient homes and care facilities to assist with daily care needs. As a valued member of our hospice team, you will help patients maintain dignity, comfort, and quality of life during their hospice journey while providing important support to families. *About the Role* As a *Certified Home Health Aide*, you will travel throughout *Long Beach/Huntington Beach* to provide hands-on personal care and supportive services to hospice patients in their homes, assisted living communities, and skilled nursing facilities. Working under the supervision of a *Registered Nurse*, you will assist patients with activities of daily living while observing and reporting changes in condition to the clinical team. Your presence provides comfort, companionship, and compassionate support to patients and families during an important stage of life. *Your Responsibilities Include* * Traveling throughout *Long Beach/Huntington Beach* to provide scheduled hospice aide visits. * Assisting patients with *activities of daily living*, including bathing, grooming, dressing, toileting, and mobility assistance. * Providing *personal care and comfort measures* according to the patient’s care plan. * Supporting patients with *light housekeeping and maintaining a safe, clean environment*. * Assisting with *patient repositioning and mobility* to promote comfort and prevent skin breakdown. * Observing patient condition and *reporting changes to the supervising RN*. * Providing *companionship and emotional support* to patients and families. * Accurately documenting patient care and visit details in the agency’s EMR system. * Maintaining *infection control practices and patient safety standards*. * Participating in communication with the interdisciplinary hospice team. This is meaningful, hands-on hospice work where you build supportive relationships with patients and families while helping ensure comfort and dignity during the hospice journey.
