Job Description
*Reliance Hospice and Palliative Care* is currently seeking a *Per Diem Bilngual Spanish Certified Home Health Aide* to support our partner locations around *San Fernando Valley*. *Hourly salary starts at $23.00.* This position provides personal care services and promotes excellent end of life care. This position requires a license in Certified Home Health Aide. _*\*Must have a CHHA experience at a Hospice Facility/Setting, not a Caregiver or CNA.*_ *ESSENTIAL FUNCTIONS:* * Gives or assists with bathing, dressing, oral hygiene, hair treatments, nails and skin care. * Assists with elimination, utilizing a bedpan or urinal, commode or bathroom. * Assists patient with eating and meal preparation including special diets prescribed for patient. * Assists with positioning, transfers and ambulation, with or without assistive devices. * Assists patient with prescribed range of motion and prescribed exercise which patient and aide have been taught by designated professional personnel. * Assists with application of non-prescription topical ointments. * Takes and records temperature, pulse, respiration and blood pressure. * Assists patient with ostomy (where function is normal and care is routine). Includes emptying pouch, changing appliance and assisting with preparation of irrigation procedure. * Charts tasks. * Communicates with patients families and related persons, and co-workers to achieve an effective exchange of verbal/nonverbal and written information. * Observe and report deviations from the usual physical and mental status of the patient and any unusual occurrence(s) to staff nurse within a timely manner. JOB QUALIFICATIONS * Must be a licensed Certified Home Health Aide (CHHA) and has CHHA experience, *required.* * 2 years' experience in hospice setting *required.* * *Must have Valid CHHA and CNA Certifications.* * CPR required * Bilingual Spanish (*preferred*) * Candidates must have prior hospice experience to be considered for this role. * Must have an outgoing personality and is also a teamplayer.
