*About us* NeoCare Palliative & Hospice (Pure Bliss Hospice Care, Inc.) is a hospice in Woodland Hills, CA that specializes in end of life comfort, for ALL ages. Our team works close with the patient, families and caregivers of the patient along with the physician to achieve seamless physical, psychosocial and spiritual care. Our work environment includes: Field Home Health Aide (CHHA) We are seeking compassionate Licensed Home Health Aide's to join our hospice and palliative care team, who's primarily responsibility is to provide our terminally ill patients with quality care. Works under the supervision of a Registered Nurse (RN), and performs various personal care services within their home environments, along with providing their loved ones with guidance and support. *Responsibilities of the Home Health Aide include, but are not limited to the following:* 1. Providing hands-on personal care including: * Baths * Back Rubs * Oral Hygiene * Shampoos * Changing bed linen * Assisting patients with dressing and undressing * Skin Care to prevent breakdown * Assisting the patient with toileting activities * Orderly, as appropriate 2. Assisting in meals & feeding, if necessary 3. Assisting in ambulation and exercise according to the plan of care. 4. Meeting safety needs of patient's and using equipment safely and properly (foot stools, side rails, etc ) 5. Reporting on patient's condition and significant changes to the assigned nurse. 6. Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct. *Qualifications:* * Must have valid/current California Home Health License/Certificate * Must have at least 1 year of recent CHHA experience in hospice, palliative care, home health, acute care &/or skilled nursing * Current CPR/BLS * Current CA driver’s license with proof of insurance, reliable transportation and willingness to drive to patient's location * Must have compassion, patience, be honest and dependable * Ability to follow and document care given Pay: $35.00 - $40.00 per hour Medical Specialty: * Hospice & Palliative Medicine * Pediatrics License/Certification: * Driver's License (Required) Willingness to travel: * 25% (Preferred) Work Location: On the road
