We prioritize dignity, independence, and excellence in every aspect of our service. *Position Summary:* We are seeking dedicated and experienced *Licensed Vocational Nurses (LVNs)* to provide private duty, one-on-one care to individuals in their homes in _*Los Angeles, CA. *_ This is an excellent opportunity for nurses who thrive in a personalized care setting, value continuity of care, and seek a meaningful relationship with patients and families. *Key Responsibilities:* * Deliver skilled nursing care to clients in accordance with physician orders and individualized care plans. * Monitor patient conditions and report changes to the RN Case Manager and physician as needed. * Administer medications and treatments as prescribed within scope of practice. * Provide assistance with ADLs (activities of daily living), including hygiene, mobility, feeding, and toileting. * Ensure proper documentation and timely charting of assessments, treatments, and interventions. * Educate the client and family on disease management, medication administration, and safety measures. * Maintain a safe, clean, and supportive home environment for the client. * Collaborate with the multidisciplinary care team to optimize outcomes and enhance the patient experience. *Qualifications:* * *Current and valid LVN license* in the state of California. * *Minimum 3 years of *LVN experience. * *Minimum 1 year of *home health, private duty, or acute care. * *Current CPR certification* (American Heart Association or equivalent). * Strong clinical and critical thinking skills. * Excellent interpersonal and communication skills. * Ability to work independently with minimal supervision. * Reliable transportation and valid driver’s license (if travel required). * Experience with tracheostomy, ventilators, or G-tube care is a plus (training available if needed). *Shift Details:* * *SET 12 HOUR SHIFT SCHEDULE: SUNDAY-WEDNESDAY 8 PM TO 8 AM AND/OR THURSDAY-SATURDAY 8 PM TO 8 AM* * Opportunities for overtime and extended case assignments. *Benefits (For Eligible Employees):* * Competitive hourly rates (DOE) * Weekly pay with direct deposit. * Flexible scheduling to support work-life balance. * Paid training and orientation. * Ongoing clinical support and supervision. * Medical, dental, and vision insurance. * Employee referral bonuses (Requirements apply) Job Type: Full-time Pay: $43.00 - $48.00 per hour Benefits: * 401(k) * Dental insurance * Flexible schedule * Health insurance * Vision insurance Application Question(s): * By submitting your application, do you consent to receiving follow up communication via text message (or SMS message) to the phone number you provided?
