Header: Duties * Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs. * Develop individualized care plans by collaborating with multidisciplinary teams, including physicians, therapists, and social workers. * Manage inpatient and outpatient cases across various settings such as hospital medicine, emergency rooms, intensive care units (ICUs), pediatrics (PICU), and post-acute care environments. * Coordinate discharge planning by evaluating post-acute needs, arranging appropriate services, and ensuring compliance with HIPAA regulations for medical records confidentiality. * Oversee utilization management processes to optimize resource use while maintaining high-quality patient care standards. * Facilitate diagnostic evaluations and medical record reviews to support accurate case documentation and treatment adjustments. * Engage in patient service activities such as intake assessments, post-discharge follow-ups, and outpatient clinic visits to promote continuity of care. Header: Skills * Extensive critical care experience with a strong background in ICU, emergency medicine, trauma centers (Level I & II), and hospital medicine. * Proven expertise in managing complex cases involving infectious diseases, behavioral health issues, geriatrics, pediatrics (PICU), and medical-surgical units. * In-depth knowledge of physiology, medical terminology, diagnostic evaluation techniques, and acute care procedures. * Skilled in discharge planning, case management strategies, utilization management protocols, and post-acute care coordination. * Familiarity with hospital experience environments including inpatient wards and outpatient clinics; adept at patient assessment and vital signs monitoring. * Strong understanding of HIPAA compliance standards along with excellent medical records documentation skills. * Effective communicator with the ability to collaborate across multidisciplinary teams while maintaining a focus on patient service excellence.
