The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performance. The home health aide will be assigned by a Registered Nurse in a manner that promotes the quality, continuity, and safety of a patient’s care. ESSENTIAL DUTIES AND RESPONSIBILITIES In addition to all components of the above position summary the following “Essential Duties and Responsibilities” are essential to successful performance in this position Monitors patient condition by observing physical and mental condition to ensure the patient's safety. Supports patients by providing housekeeping and laundry services, shopping for food, and other household requirements, such as preparing and serving meals and snacks. Assists with feeding the patient during meal servings and/or snacks. Helps patients by providing personal services, such as bathing, dressing, grooming, laundry, and self-care. Assist patients with medication reminders and administration, if required. Facilitates patients' care for themselves by teaching the use of a cane or walker, special utensils to eat, and special techniques and/or equipment for personal hygiene. Helps with any daily activities and exercise according to the plan of care assign. Assist family members with the care of the patient by teaching appropriate ways to lift, turn, and re-position if needed. Records patient information by making entries in the patient journal and notifying the nursing supervisor of changing or unusual conditions. Maintains a safe, secure, and healthy patient environment by following asepsis standards and procedures, maintaining security precautions, and following prescribed dietary requirements and nutrition standards. Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements. SUPERVISORY RESPONSIBILITIES The incumbent in this position does not have any direct supervision. EDUCATION/EXPERIENCE/CREDENTIALS Must have a current California Certified Home Health Aide Certificate Minimum 1+ year(s) of professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors. Possesses and maintains current CPR/First Aid certification and negative TB skin test or chest x-ray. Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record. Willing and able to work in the San Gabriel Valley area/ Arcadia area KNOWLEDGE, SKILLS, AND ABILITIES To perform this position successfully, you must be able to perform each essential duty and responsibility satisfactorily.
