*Job Summary* We are seeking a compassionate and dedicated Registered Nurse - Home Health to provide high-quality patient care in the comfort of their homes. The ideal candidate will possess strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team. This role involves delivering nursing care, educating patients and their families, and ensuring adherence to treatment plans. *Duties* * Conduct comprehensive patient assessments and develop individualized care plans. * Administer medications and treatments as prescribed, including ostomy care and tube feeding. * Monitor patient progress and report any changes in condition to the healthcare team. * Maintain accurate medical records using eClinicalWorks software. * Educate patients and families on disease management, medication administration, and home community care practices. * Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care. * Provide support for outpatient procedures such as cardiac catheterization when necessary. * Participate in quality improvement initiatives to enhance patient outcomes. *Experience* * Current Registered Nurse (RN) license in the state of practice. * Previous experience in a clinic or teaching hospital setting is preferred. * Familiarity with home health care practices and standards is highly desirable. * Proficient in using electronic medical records systems, particularly eClinicalWorks. * Experience with cardiac catheterization, ostomy care, and tube feeding is a plus. * Strong communication skills and the ability to teach patients effectively. * Ability to work independently while managing multiple priorities in a home health environment.
