*Job Overview* We are seeking a compassionate and dedicated Hospice Field Registered Nurse to join our dynamic healthcare team. In this vital role, you will provide expert nursing care to patients in their homes or community settings, ensuring comfort, dignity, and quality of life during their end-of-life journey. This position offers an exciting opportunity to make a meaningful difference while working in a flexible, patient-centered environment. *Duties* * Conduct comprehensive patient assessments to evaluate physical, emotional, and spiritual needs in accordance with hospice care protocols * Develop and implement individualized care plans that promote comfort and symptom management * Administer medications, treatments, and interventions as prescribed by physicians, ensuring safety and efficacy * Educate patients and families on disease processes, medication management, and end-of-life care options * Coordinate with interdisciplinary teams including physicians, social workers, chaplains, and therapists to ensure holistic patient support * Monitor patient progress through regular vital signs checks and clinical observations, adjusting care plans as needed * Document all patient interactions accurately in medical records while adhering to HIPAA regulations *Requirements* * Valid RN license with current registration in the state of practice * Critical care experience or background in Level II trauma centers highly preferred * Experience in ICU, emergency medicine, or hospital medicine settings demonstrating strong acute care skills * Knowledge of physiology, medical terminology, and diagnostic evaluation techniques essential for effective patient assessment * Familiarity with discharge planning, case management, utilization management, and medical records documentation * Ability to manage complex cases involving infectious disease care, geriatrics, behavioral health or pediatrics is a plus * Excellent communication skills for patient education and team collaboration * Strong organizational skills with the ability to prioritize tasks efficiently in a fast-paced environment Join us to provide compassionate end-of-life care that truly makes a difference.
