Whole Health Pharmacy, located in Santa Ana, CA, strives to promote and achieve optimal therapeutic outcomes through collaborative Disease Therapy Management. Our goals include successfully improving the quality of life for each Patient, providing the highest standard of clinical services, reducing overall healthcare costs, and fostering relationships between Patients, Providers, and Payers. Seeking a per diem (approximately 20-25 hrs per week) highly motivated, team-oriented, patient-focused Infusion Nurse to support our Santa Ana, CA pharmacy location. In addition to on site support, the infusion nurse will also make patient visits in the home. Bilingual Vietnamese or Spanish-speaking preferred. The Infusion Nurse is responsible for coordinating and providing direct patient care in alternate treatment sites (patient homes) and, on an occasional basis, at the Santa Ana office infusion suites while communicating and collaborating with the pharmacists, physicians, and the health care team. The Infusion Nurse administers medication while ensuring patient safety and compliance under the physician’s plan of care. Responsibilities of the Infusion Nurse include the following: 1. Assess, initiate and develop nursing plans of treatment 2. Evaluate patient progress towards treatment plan goals 3. Mix and prepare medication once an IV line is established for patient, per infusion order or manufacturer recommendations 4. Administer IV infusions and medications, adhering to best practices and infusion orders to ensure patient safety and positive outcomes 5. Educate patients and/or caregivers on infusion process and medication side effects 6. Assess patient needs and physical status at each visit with proper documentation 7. Effectively communicate any changes or updates in the patient’s status to the pharmacy team, medical provider, patient, and/or caregivers 8. Prepare clinical documentation during visits and submits documentation to comply with established policies and practices 9. Report documentation in a timely manner, following established policy 10. Administer infusion via gravity, rate flow, or pumps provided by the pharmacy 11. Report any misconduct, suspicious or unethical activities to the Compliance Officer 12. Represent the pharmacy in a professional and courteous manner in all interactions with patients 13. Assist with the resolution of patient complaints and concerns in a professional manner 14. Document and report patient concerns and complaints to the Pharmacy team 15. Report incidents in compliance with pharmacy policy 16. Participate in surveys conducted by authorized inspection agencies 17. Participate in the pharmacy’s Quality Management Program as requested by the Compliance Officer 18. Participate in pharmacy committees when requested 19. Participate in in-service education programs provided by the pharmacy 20. Assist in the implementation of policies and procedures within the pharmacy 21. Maintain confidentiality of work-related issues 22. Perform other duties as assigned by supervisor Minimum Qualifications 1. RN License or Equivalent 2. Knowledge of rare disease/specialty infusion therapies 3. Organizational skills sufficient to maintain consistently accurate records 4. Ability to evaluate options and make efficient decisions 5. Computer skills that include proficiency with spreadsheets, use of the internet, and email Preferred Qualifications: Bilingual Spanish or Vietnamese Physical Requirements The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions in this job description: 1.
