*Overview:* Chelsea Home Health Care is seeking experienced and compassionate Licensed Vocational Nurses (LVNs) to provide one-on-one care for a *2-year-old female* patient in a home setting. This case requires skilled nursing care, including G-tube, tracheostomy, and ventilator support. *1:1 care. * *Our Philosophy:* Our philosophy is rooted in compassion, clinical excellence, and growth. Through hands-on experience and ongoing guidance, we help you expand your skills, strengthen your knowledge, and grow both personally and professionally. *Location:* Downey / Santa Fe Springs *Schedule:* Monday – Friday *(Morning & Grave yard shift)* * 7:00 AM – 3:00 PM * 11:00 PM – 7:00 AM _*(Hiring for 2 positions – one per shift)*_ *Duties* * Provide skilled pediatric nursing care in the home * Perform *G-tube feedings and care* * *Provide tracheostomy care and suctioning* * *Manage and monitor ventilator support* * Monitor patient status and respond to changes in condition * Ensure patient safety, comfort, and well-being * Maintain accurate documentation and care notes * Communicate effectively with family and healthcare team * Follow physician orders and individualized care plan *Requirements* * Active California LVN license in good standing * Pediatric experience preferred * Experience with G-tube, tracheostomy, and ventilator care required * Current CPR/BLS certification (Pediatric preferred) * Strong clinical and observational skills Compassionate, reliable, and detail-oriented *Why Join Us:* * One-on-one patient care * Consistent weekday schedule * Supportive and professional team * Competitive pay *About Us:* At Chelsea Home Health Care, we believe in making a meaningful difference in the lives of our patients and the community we serve.
