Our mission is to deliver excellence through trust, communication, and support—helping our patients heal with confidence and dignity. *Position Summary* We are seeking a *Licensed Vocational Nurse (LVN)* to join our growing home health team. The LVN will provide direct nursing services to patients in their homes in accordance with agency standards, physician orders, and state regulations. This role ensures patients receive safe, effective, and compassionate care while supporting the Registered Nurse and interdisciplinary team. *Responsibilities* * Provide direct skilled nursing care to patients in their homes under the supervision of a Registered Nurse. * Administer medications and treatments as prescribed by the physician. * Document all care and communication accurately in compliance with agency and state standards. * Educate patients and families on health maintenance, medications, and disease management. * Monitor patient condition and promptly report changes to the supervising RN and physician. * Promote safety, infection control, and proper use of home health equipment. * Maintain effective communication with patients, families, and interdisciplinary staff. * Participate in case conferences and staff meetings as required. * Perform other related duties as assigned to support quality patient outcomes. *Qualifications* *Required:* * Current *California LVN license* * Valid *Driver’s License* and reliable vehicle with insurance * Minimum of *6 months of LVN experience* (Home Health preferred) * Strong communication, organization, and clinical assessment skills * Ability to lift up to 50–75 lbs and travel up to 100 miles per day *Preferred:* * IV and Blood Withdrawal Certification * Two (2) years of home health or related healthcare experience * Knowledge of home health documentation and EMR systems *Required Credentials (must remain current):* * LVN License * Driver’s License & Auto Insurance * TB Test / Chest X-Ray * Physical Exam * CPR Certification Pay: $60,000.00 - $80,000.00 per year Application Question(s): * Are you willing to travel within a 20–30 mile radius to visit patients in their homes? * Do you have a minimum of 6 months of LVN experience? * Do you have at least 2 days of availability per week for patient visits?
