Job Description
*Job Summary:* The Part-Time On-Call LVN provides skilled nursing care and support to hospice patients during after-hours and weekends. *Responsibilities:* * Respond promptly to on-call assignments for hospice patients during evenings, weekends, and holidays. * Assess patient needs and provide appropriate nursing interventions in accordance with the plan of care. * Communicate effectively with patients, families, caregivers, and interdisciplinary team members. * Document all nursing care provided accurately and in a timely manner. * Collaborate with the RN Case Manager and Hospice Medical Director as needed. * Provide emotional support and education to families and caregivers. * Maintain compliance with all federal, state, and local regulations as well as company policies and procedures. *Qualifications:* * Current LVN license * Minimum of 1 year of nursing experience; hospice or palliative care experience preferred. * Ability to work independently and respond calmly under pressure. * Strong communication and critical thinking skills. * Compassionate, dependable, and dedicated to quality patient care.
