Five Star Home Hospice is a well-established hospice agency located in Pasadena, CA that is seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to join our growing team. *Position Overview:* As a Visiting LVN, you will provide compassionate, patient-centered care to individuals receiving hospice services in their homes or residential facilities. Full-time LVNs are primarily assigned to patients in the *San Gabriel Valley/San Fernando Valley*, with assignments planned to support efficient routing and minimize travel. You will manage a structured caseload, with patients receiving an average of *3 visits per week* based on individualized care plans (ranging from 1–7 visits depending on acuity). LVNs typically complete *3–4 visits per day*, allowing for a balanced and manageable workload while maintaining high-quality care. Each visit lasts approximately one hour, giving you the time to build meaningful connections with patients and families. Visits are coordinated in advance and generally scheduled between *9:00 AM and 5:30 PM*.
