Job Description

We are looking for a compassionate Home Health Aide, RN, and LVN to provide assistance in daily living, personal care services, and skilled nursing care in patients with chronic diseases at home. The ideal candidate will be patient and professional and will be able to follow instructions and work safely under the direction of a healthcare provider. *Responsibilities* * Provide personal care and services such as bathing, grooming, feeding, dressing, etc. * Assist with daily personal care (including but not limited to) * IV, wound care, and other skilled nursing needs * Provide companionship and support * Report changes in patient condition or behavior. * Document and sign reports. * Work with other team members. * Do not make changes in patient’s condition or behaviors without the consent of the patient. * Be a team player! Experience: * Home Health Aide: 1 year (Preferred) * No experience is okay * 2nd language a plus!
