*Job Overview* We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide compassionate patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations through proactive health management and skilled clinical support. *Duties* * Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. * Administer medications and injections accurately following physician orders while monitoring for adverse reactions. * Manage support to optimize patient comfort and safety. * Document all patient interactions meticulously using EMR (Electronic Medical Record) systems such ensuring compliance with HIPAA regulations. * Assist with diagnostic evaluations like laboratory specimen collection, phlebotomy, and medical imaging preparations. * Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments in memory care or hospice settings. * Support emergency response efforts by managing airway issues, performing basic life support (BLS), and participating in acute pain management protocols. * Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care to ensure seamless patient transitions. *Experience* * Valid LVN license with current state certification required; additional certifications in Basic Life Support (BLS) strongly preferred. * Demonstrated experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, or outpatient clinics. * Hands-on experience with ICD-9/10 coding for accurate documentation and billing processes. * Prior work with populations requiring specialized care such as pediatrics, neonatal ventilators, or patients with infectious diseases or complex chronic conditions. * Knowledge of medical terminology, anatomy and physiology principles, and diagnostic evaluation techniques essential for effective patient assessment. * Experience administering injections, performing venipuncture or catheterization procedures with aseptic technique. * Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks to ensure precise medical documentation and efficient workflow management.
