Hospice Registered Nurse – Admissions, Evaluations, Pre-Evaluations & EOB ( Part Time & PerDiem)

Job Description

We are seeking a compassionate and detail-oriented Hospice Registered Nurse (RN) specializing in Admissions, Evaluations, and Pre-Valuations. In this role, you will serve as the first clinical point of contact for patients and families, ensuring a seamless transition into hospice care. You will conduct pre-admission evaluations, determine eligibility, and guide patients through the Election of Benefits process with clarity and empathy. *Key Responsibilities* * Conduct pre-valuations (pre-admission assessments) to evaluate patient eligibility for hospice services * Review medical records, physician referrals, and clinical criteria to support eligibility determination * Perform comprehensive admissions and initial nursing assessments in homes, facilities, or hospitals * Facilitate and explain the *Election of Benefits (EOB)*, ensuring patients and families understand hospice services, coverage, and rights * Obtain all required consents and documentation in compliance with regulatory standards * Collaborate with physicians and the interdisciplinary team to establish individualized plans of care * Educate patients, families, and caregivers on hospice philosophy and expectations * Ensure accurate, timely, and compliant documentation in EMR systems * Coordinate with referral sources, case managers, and discharge planners to ensure smooth transitions * Provide emotional support and guidance during a critical and sensitive time *Qualifications* * Active Registered Nurse (RN) license in California * Minimum of 2-3 years of nursing experience (hospice, home health, or acute care preferred) * Strong clinical assessment and critical thinking skills * Knowledge of hospice eligibility criteria and regulatory requirements * Excellent communication and interpersonal skills * Ability to work independently and manage time efficiently * Valid driver’s license and reliable transportation * Prior hospice admissions or intake experience * Familiarity with Medicare/Medicaid hospice regulations and documentation standards * Experience with electronic medical records (EMR) systems Pay: From $50.00 per hour Work Location: In person
