ABOUT THE ROLE Los Robles Hospice is looking for a clinically skilled and relationship-driven professional to serve as our Field Hospice Clinical Supervisor – Facility Partnerships. This is a hybrid clinical leadership and relationship-management role that puts you at the center of our facility partnerships — the heartbeat of how we grow and deliver exceptional hospice care. You'll spend your time in the field building trusted, lasting relationships with Board & Care homes, Assisted Living (AL), Independent Living (IL), Skilled Nursing Facilities (SNFs), and hospitals — serving as the clinical face of Los Robles Hospice within these partner environments. *This is a clinical leadership role that works in close partnership with the DPCS. As a large agency with extensive facility relationships, our goal is to elevate the level of concierge-style care delivered within these settings. Strengthening clinical quality and responsiveness in facilities directly supports and reinforces our strong business development efforts.* *Incentives are tied to the overall health and stability of facility referral flows. To be clear, this is not a sales position. It is a quality-driven role focused on clinical excellence, relationship stewardship, and maintaining a consistently high standard of care within our partner facilities.* WHAT YOU'LL DO Facility Relationship Development * Conduct regular rounding at key partner facilities * Build strong working relationships with Directors of Nursing, Administrators, Case Managers, Social Workers, and Discharge Planners * Provide clinical education on hospice eligibility, transitions of care, and symptom management * Address facility concerns promptly — be the most responsive hospice partner they've ever had Clinical Support & Oversight * Coordinate care between hospice teams and facility staff * Resolve clinical concerns, escalation issues, and service gaps * Participate in complex case discussions and reinforce care plans with facility teams Marketing & Business Development Collaboration * Partner with our marketing and outreach team to identify education and engagement opportunities * Support facility in-services and hospice education sessions * Help remove operational or clinical barriers to referrals * Maintain visibility and trust within high-potential facilities Concierge & Patient Experience * Support smooth, compassionate patient transitions to hospice * Ensure families feel cared for at every step of the admissions process * Collaborate with our Concierge team to resolve logistical needs quickly Admission Support (As Needed) * Step in to complete hospice admissions when coverage gaps arise or urgent facility needs require it WHO YOU ARE * An RN (preferred) with hospice or palliative care experience * Experienced working with SNFs, Assisted Living, or hospital discharge teams * A natural relationship builder who earns trust through clinical credibility and follow-through * Highly responsive — you don't let things fall through the cracks * Solutions-oriented and comfortable working independently in the field * Deeply knowledgeable about hospice eligibility and transitions of care WHY LOS ROBLES HOSPICE Facilities need a hospice partner who shows up — one that responds quickly, communicates clearly, supports their staff, and delivers excellent patient care.
