*Part Time 17.5 hours Mon-Fri 9:30am-1pm* *Licensed Vocational Nurse (LVN) * *A Step to Freedom, Inc.* *Position Summary* The Licensed Vocational Nurse (LVN) provides compassionate, client-centered nursing care within A Step to Freedom’s Interim Housing Program. The LVN supports residents experiencing homelessness, mental health challenges, substance use disorders, and chronic medical conditions by delivering direct care, medication support, health education, crisis response, and coordination with internal and external providers. This position works under the direction of the Program Manager and with oversight from the Nursing Supervisor (when applicable), in alignment with all county and contractual compliance standards. *Key Responsibilities Clinical Care & Monitoring* * Conduct daily wellness checks and assess changes in physical or mental health. * Assist with chronic disease management (e.g., diabetes, hypertension, asthma). * Provide injections, wound care, dressing changes, vital signs, TB assessments, and basic first aid. * Support residents with medication monitoring, logging, counting, and protocol compliance. * Complete accurate and timely nursing documentation, including progress notes, MAR reviews, incident reports, and medication logs. *Medication Support & Compliance* * Oversee medication storage, handling, and distribution per county regulations and ASF protocols. * Review MARs daily, ensuring accuracy and correcting discrepancies. * Coordinate medication refills and delivery with pharmacies, hospitals, and external providers. * Educate residents on medication usage, side effects, and adherence. * Notify the Program Manager and RN of any irregularities, refusals, errors, or health concerns. *Health Education & Advocacy* * Provide residents with education on hygiene, chronic illness management, nutrition, harm reduction, and general wellness. * Encourage and assist residents in scheduling and attending medical appointments. * Advocate for residents' access to medical, dental, and behavioral health services through county partners and community clinics. *Crisis Response & Safety* * Respond promptly and professionally to on-site medical emergencies and behavioral health incidents. * Conduct risk assessments and collaborate with Program Managers, Case Managers, and on-call supervisors to ensure the safety of residents and staff. * Contribute to the development and implementation of individualized health plans for high-need residents. * Report all incidents, injuries, emergencies, or unusual behaviors according to ASF policy. *Collaboration & Communication* * Work collaboratively with Program Managers, Case Managers, Resident Aides, and external service partners. * Participate in team meetings, interdisciplinary case conferences, and daily coordination. * Maintain strong communication with multidisciplinary partners, including hospitals, ICMS teams, mental health providers, LAHSA, and other county entities. * Follow all HIPAA and ASF confidentiality standards. *Compliance & Documentation* * Ensure all nursing activities meet contract requirements, DHCS standards, and ASF internal policies. * Complete nursing logs, assessments, incident reports, and shift documentation in a timely and accurate manner. * Participate in audits, chart reviews, and compliance inspections. * Support consistent medication room procedures and ensure adherence by all staff. *Qualifications* * Valid California LVN license in good standing (required). * Current CPR/BLS certification (required). * Experience working with homeless populations, behavioral health, or medical-surgical settings preferred. * Knowledge of trauma-informed care and harm reduction practices. * Strong communication, documentation, and organizational skills. * Ability to work independently and manage multiple priorities in a fast-paced environment. * Ability to remain calm and professional in crisis situations. *Work Environment* * Interim Housing facility serving adults experiencing homelessness. * Requires standing, walking, and occasional lifting of up to 25 lbs. * Exposure to behavioral health crises, medical emergencies, and fast-paced, high-stress scenarios. *Compensation* *Pay Rate:* $30 per hour *Status:* Part Time *Join Our Team* Become part of A Step to Freedom, Inc. and help make a meaningful impact in the lives of individuals transitioning out of homelessness.
