*Position: *Registered Nurse Case Manager(Part Time/Per Diem) *Reports to: *Director of Patient Care Services *Job description:* We are looking for dedicated RNs who want to provide quality care to home based patients. You will work one-on-one with the patient and truly impact their life as well as their families. The job is consistent, long-term, and can work with a variety of different schedules. The RN is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. *This is an office-based position. The RN Clinical Supervisor will be overseeing LVN patient home care which includes Patient home visits throughout Northern LA County areas(Antelope Valley, Santa Clarita Valley, and San Fernando Valley), creating and updating Plans of Care, training and orienting LVNs, and conducting initial patient assessments.* *ABOUT PRIME HOME HEALTH (Prime HH)* Prime Home Health in-home nursing care for patients.
