Job Description
Key Responsibilities: Provide direct skilled nursing care according to physician’s orders and the plan of care Perform wound care, injections, medication administration, IV support (if applicable) Monitor patient vitals, document assessments, progress notes, and coordinate with case managers / supervisors Educate patients and families / caregivers on care plans, disease processes, home safety, etc. Travel to patient homes within assigned territory; maintain charting & documentation accurately and timely Qualifications: Current California LVN license in good standing At least 1 year of nursing experience; home health experience preferred Valid CA driver’s license, reliable transportation, auto insurance CPR certification Strong communication skills; bilingual skills a plus (e.g. Spanish, Vietnamese, etc.) Ability to work independently; comfortable with home visits
