Join Our Team as a Licensed Vocational Nurse (LVN) Where Compassion Meets Opportunity Anchor Co-Living is seeking a couple of dedicated Licensed Vocational Nurse (LVN) team members to join our Recuperative Care & Housing team serving individuals recovering from illness, injury, or hospitalization who would otherwise have nowhere safe to heal. This role blends clinical care with compassion - helping residents stabilize medically while connecting them to housing, primary care, and community supports under California's CalAIM Community Supports initiative. We believe that every patient deserves more than a bed - they deserve stability, care, and a pathway forward. Here, your nursing expertise will directly shape their recovery journey. You'll work in a non-acute, residential setting - not a hospital - with a small caseload and a supportive interdisciplinary team that includes RNs, care coordinators, and case managers. Why This Role Matters Our Recuperative Care LVNs play a pivotal role in ensuring safe transitions from hospitals to community-based housing. Working as part of a multidisciplinary team - including RNs, case managers, social workers, and housing navigators - you'll help deliver high-quality, trauma-informed care in a supportive residential environment. You'll help clients regain independence, adhere to care plans, and connect to ongoing medical and social supports - all while maintaining compliance with Medi-Cal and managed care plan standards. What You'll Be Doing Administer medications, perform wound care, and monitor vital signs Provide support for daily living activities with compassion and respect Identify changes in patient condition and report to supervising clinicians Maintain accurate, timely documentation in compliance with standards Promote a safe, clean, and positive care environment What You Bring Active LVN license in the state of California (in good standing) Current CPR/BLS certification 2+ years of experience in direct patient care (home health, skilled nursing, or similar) Experience with Alzheimer's, dementia, or chronic illness management a plus Physically capable of assisting patients with mobility and transfers Positive attitude, emotional resilience, and team-first mindset What You'll Get Competitive Pay: $33-$36 per hour Supportive Culture: You'll be part of a collaborative team that values your input Work-Life Balance: Flexible part-time schedule Time to Recharge: Paid time off available Growth Potential: Opportunity to expand your role and take on more responsibility Location On-site in Los Angeles, CA 90007.
