If you are looking for a company that will treat you like family, then Acacia is for you. www.acaciahealth.net Responsibilities: Oncall hospice visits on weekends Provide accurate and timely documentation Assess and provide patient and family/caregiver education Perform other duties as assigned by the registered nurse Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc. Qualifications: Demonstrated compassion and commitment to patient centered care Active LVN license in the state of California Valid California driver’s license and reliable transportation Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care Prior experience in nursing or related medical fields Experience as an on call LVN runner preferred Job Types: Per diem, PRN Schedule: Weekends Nights (Saturday and Sunday) 6p8:30a Service Coverage: San Gabriel Valley About Acacia Hospice of Southern California LLC The Acacia Promise!
